Sun River Health is currently seeking a full-time Patient Care Center Registered Nurse to join our team.<br><br><strong>Job Type:</strong> Full Time <br><br><strong>Pay: </strong>$45.00/hr - $52.10/hr <br><br><strong>Sign On Bonus:</strong> $2,700 (Conditions Apply)<br><br><strong>Summary: </strong><br><br>The PCC Registered Nurse is a licensed nurse responsible for delivery of nursing care remotely to individual clients. The Registered Nurse maintains the quality of client care through the ongoing processes of assessment, planning and implementation of nursing activities through the Patient Care Center.<br><br><strong>Responsibilities: </strong><br><ul> <li>Meets good customer service to ensure quality call handling</li> <li>Follows call center nursing and scheduling workflows</li> <li>Assist staff with Urgent and Red Flags patient's health concerns.</li> <li>Support site level Prescription Refill and Medical Call Buckets during scheduled outbound call times</li> <li>Develops, implements and evaluates the nursing care regimens for clients.</li> <li>Plans, coordinates and assesses client care.</li> <li>Identifies client needs and provides remote nursing care based on the client's care plan.</li> <li>Collaborates with interdisciplinary team members to develop, implement and evaluate client care.</li> <li>Meets call handling performance</li> <li>Functions as preceptor to students, new hire candidates and other clinical team members/ staff when needed</li> <li>Assists with Quality Assurance audits as assigned.</li> <li>Identifies quality issues and opportunities for improving quality of care.</li> <li>Documents client care with accurate and accessible records.</li> <li>Communicates with Pharmacy, Laboratories and other vendors as needed.</li> <li>Assists with medication refills per Sun River Health protocols</li> <li>Maintains professional competence through participation in staff development and educational program.</li> <li>Provides clinical education and counseling to patients for health promotion and disease prevention to improve health outcomes</li> <li>Have an appropriate and quiet remote work environment, as well as supplying and maintaining good Internet Connectivity required for Call Center Platforms</li></ul><br><strong>Requirements: </strong><br><ul> <li>Bachelors Degree in Nursing Required </li> <li>New York State Registered Nurse license </li> <li>CPR/BLS -HCP Required </li> <li>Bilingual Preferred</li></ul><br><br><strong>About Sun River Health</strong><br><br>Sun River Health has a rich history of providing quality, affordable health care to communities in need.
