Nurse Practitioner – In-Home & Telehealth Care

<p>Position Overview</p><p>ACE Physicians is expanding into Dallas Fort Worth and we are looking for compassionate and motivated Nurse Practitioners to help bring high-quality, patient-centered care directly to the communities we serve.</p><p>ACE Physicians is the sister company of <strong>Varco Healthcare</strong>, an established healthcare organization focused on delivering innovative and accessible care models. Together, our organizations are committed to improving healthcare access and outcomes through patient-centered services.</p><p>This hybrid role provides care through <strong>in-home visits and telehealth</strong>, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable. This is an exciting opportunity to join a growing organization and play a key role in establishing our presence in a new region.</p><p>We are seeking clinicians who are passionate about improving access to care, working with diverse patient populations, and delivering thoughtful, individualized treatment.</p><p>If you enjoy flexibility, autonomy, and the opportunity to help shape healthcare in a growing organization, we would love to hear from you.</p><p>Key Responsibilities</p><ul><li>Perform comprehensive health assessments and physical examinations during <strong>in-home visits and telehealth appointments</strong></li><li>Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care</li><li>Develop and implement individualized treatment plans tailored to each patient’s needs</li><li>Educate patients and families on health management, wellness, and disease prevention</li><li>Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care</li><li>Perform minor clinical procedures such as wound care as appropriate during home visits</li><li>Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms)</li><li>Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities</li></ul><p>Qualifications</p><ul><li>Active, unrestricted Nurse Practitioner license (required)</li><li>Experience in geriatrics, internal medicine, or primary care preferred</li><li>Previous home health or home-based care experience strongly preferred</li><li>Experience caring for patients with disabilities is highly desirable</li><li>Comfortable delivering care in both <strong>in-home and telehealth settings</strong></li><li>Excellent communication skills and a collaborative, team-oriented mindset</li></ul><p>Medical Specialties</p><ul><li>Geriatrics</li><li>Internal Medicine</li><li>Primary Care</li></ul><p>Why Join ACE Physicians?</p><p>This is a unique opportunity to join ACE Physicians as we expand into DFW.
