Licensed Vocational Nurse - Field Nurse

Job Summary

We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, delivering exceptional patient care in diverse community settings. In this vital role, you will provide skilled nursing services directly in patients' homes and facilities, ensuring comprehensive assessments, medication administration, and personalized care plans. Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using Kinnser (WellSky) systems.
  • Administer medications, injections, IV infusions, and perform catheterizations following strict aseptic techniques and HIPAA guidelines to ensure safety and compliance.
  • Provide specialized care for individuals with developmental disabilities, memory impairments, or those in assisted living environments.
  • Assist with wound care, tube feeding, ventilator management, and other advanced procedures like spinal taps or gastric lavage as needed.
  • Educate patients and families on health coaching topics including medication management, nutrition, infection control, and discharge planning to promote independence and well-being.
Experience
  • Valid LVN license with current state certification required; must be in good standing.
  • Demonstrated experience in various healthcare settings such as long-term care facilities, nursing homes, or home health environments.
  • Prior work with diverse patient populations including pediatrics, geriatrics, behavioral health patients or individuals with disabilities is highly valued.
  • Strong assessment skills in triage situations; ability to handle urgent care scenarios confidently while maintaining compassionate patient interactions.
