We are seeking a compassionate and self-motivated Nurse Practitioner (NP) to join our innovative care team that includes engagement with the flexibility of virtual care.

This role will require ongoing virtual care management. The ideal candidate is comfortable delivering care via telehealth platforms and values building long-term patient relationships through  a mix of home-based and virtual interactions. 

What You'll Do:

  • Conduct  in-person initiating visits for Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services
  • Provide virtual follow-up visits and chronic condition support
  • Collaborate with nurses, specialists, and pharmacists as part of a broader care team
  • Document clinical findings and ensure compliance with Medicare and state guidelines
  • Participate in patient education, care plan review, and data-informed decision making

Requirements:

  • Active Nurse Practitioner license in Florida AND Mississippi
  • Minimum of 3 years of NP experience
  • DEA license required
  • Comfortable providing care via telehealth and documenting in EHR systems
  • Strong communication and orgaizational skills
